How do I update my name or date of birth?

In order to update the personal information on your profile, including name or date of birth, please send two forms of identification to [e-mail]. 

Acceptable forms of identification include a driver’s license, passport, social security card, birth certificate, government identification card or military identification.

The identification documents must include all relevant information that you are requesting to update (i.e., first name, middle name, last name).

Users may update their mailing address, etc., by editing their profile details. To do that, click on the person icon and "View Profile".
